Procedure Summary

The drug test result processing in the lab includes four core steps: Accessioning, Screening, Extraction, and Confirmation.

Any samples that test positive during Screening must undergo a secondary confirmation process.

Should a sample test positive during Screening, additional hair is taken from the primary sample for Extraction. In this phase, drugs are extracted at far lower concentrations compared to methods like urine or oral fluid testing, making hair drug screening especially challenging.

Confirmation for any positive screening outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. Any positive samples identified during Screening are cleaned before confirmation if required. The entire process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair guidelines and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
