What it is, How it works

Hair testing is increasingly regarded as an effective means for identifying drug and alcohol consumption. Hair strands capture biomarkers within their fibers, offering a prolonged history of substance use. When gathered near the scalp, hair offers detection for alcohol and other substances for up to three months. Collection is straightforward, it's tricky to tamper with, and easy to transport.

A 1.5-inch segment of roughly 200 hair strands (approximately the width of a #2 pencil) near the scalp offers 100mg of hair, the optimal amount for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advised. Using a jeweler's scale is suggested for weighing the specimen. If scalp hair isn't available, an equivalent amount of body hair can be used. In this context, head hair refers strictly to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four crucial steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is integrated into a lab's system. It involves confirming the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary additional data entry beyond what's given by an electronic chain of custody system.

Screening entails an initial rapid check for substances of abuse. While this step is cost-effective for eliminating drug use in most samples, a positive result must be confirmed for legal acceptability. Any samples initially testing positive in Screening require further confirmation.

Should the Screening stage yield a presumptive positive result, additional hair is extracted from the original sample and prepared for Extraction. At this stage, hair releases drugs at lower concentrations compared to other methods (like urine or oral fluid), making hair drug screening challenging.

Confirmation of any positive screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives are cleaned before confirmation as necessary. The entire laboratory process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
