If scalp hair isn't available, an equivalent amount of body hair is acceptable. Reference to head hair pertains strictly to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial processing phase of incorporating a sample into a lab’s system. This phase ensures the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not provided by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient technique to exclude drug use in most samples, a positive screen demands confirmation for court admissibility. Any samples that yield a presumptively positive outcome in Screening necessitate subsequent confirmation.

If the Screening stage results in a presumptive positive, additional hair is extracted from the initial sample and readied for the Extraction phase. Here, drugs are isolated from the hair at far lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening poses the highest degree of difficulty to perform.

Positive results from Screening are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as required. The entire lab process, from Accessioning to Confirmation, is reviewed under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
