A 1.5-inch segment of roughly 200 strands of hair (similar to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal quantity for evaluation and validation. For EtG, additional tests, and/or assessments beyond 10 panels, 150mg of the specimen is advised. Measuring the specimen on a jeweler's scale is recommended. If scalp hair is not available, a corresponding quantity of body hair can be gathered. When referring to head hair, it pertains solely to scalp hair. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The primary four phases in the laboratory processing of a drug test result encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a specimen into a laboratory's system. This step includes verifying that the sample was appropriately sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening incorporates an initial brief analysis for drugs of abuse. While Screening is a cost-efficient method to rule out drug use on most samples, a positive screen requires confirmation to be acceptable in court. Any samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a specimen appears presumptively positive in the Screening phase, more hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at a considerably lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to perform.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as required. The entire laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
