Collected near the scalp, it can offer up to a 3-month detection timeframe for alcohol and drugs. Hair is straightforward to gather, hard to tamper with, and simple to dispatch.

A sample of about 1.5 inches containing approximately 200 hairs (comparable to the thickness of a #2 pencil) closest to the scalp yields 100mg, the perfect sample size for both screening and confirmation. For EtG, additional tests, or panels exceeding 10, a 150mg quantity of the sample is suggested. We advise using a jeweler’s scale to measure the sample. If scalp hair is not accessible, a similar quantity of body hair might be gathered. By head hair, we mean scalp hair; body hair refers to all other types (like facial and axillary).

Process Overview

The primary stages in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the lab’s system. This step verifies if the sample was sealed and shipped correctly, assigns a LAN (Laboratory Accessioning Number) randomly, and finishes any extra data entry not given by an electronic chain of custody system.

Screening represents an initial rapid assessment for drugs. While it's a cost-efficient approach to exclude drug usage in most samples, a positive result requires confirmation to be valid in court. Samples that are presumptive positive in Screening require further confirmation.

If a sample shows presumptive positivity in Screening, more hair is extracted from the initial sample for the Extraction phase. Here, drugs are extracted at lower concentrations compared to other methodologies like urine or oral fluid, making hair drug screening one of the toughest techniques to execute.

Confirmation of any positive screen is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation as necessary. The entire processing from Accessioning to Confirmation is overseen by both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
