If scalp hair cannot be collected, a comparable amount of body hair may be utilized. "Head hair" pertains exclusively to scalp hair, whereas "body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

The procedure in laboratories for processing drug test outcomes includes four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of the sample within a lab’s infrastructure. This process includes the verification of proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any outstanding data entry absent from an electronic chain of custody system.

Screening entails a preliminary rapid assessment for substances of abuse. While Screening is a cost-efficient approach for excluding drug use in most samples, a positive screening must be validated for legal acceptability. Samples that are preliminarily positive in Screening necessitate further confirmation.

If Screening indicates a potential positive, additional hair is taken from the original sample for Extraction. During Extraction, substances are isolated from hair at lower concentrations than other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening techniques.

Positive Screening outcomes are validated through GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive samples undergo washing before confirmation as required. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
