A sample size of 1.5 inches consisting of approximately 200 hairs (almost akin to the thickness of a #2 pencil) nearest to the scalp delivers 100mg of hair, the optimal quantity for testing and verification. For tests including EtG, additional checks, and/or those beyond 10 panels, 150mg of hair is advised. We suggest using a jeweler’s scale to measure the specimen. If scalp hair cannot be obtained, a corresponding amount of body hair may be used. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other hair forms (beard, underarms, etc.).

Process Overview

The primary phases in the lab processing of drug test outcomes are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample upon entry into a laboratory’s framework. This step involves ensuring that the sample was duly sealed and transported, assigning it a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent in an electronic chain of custody configuration.

Screening presents a budget-friendly approach to exclude drug utilization in most samples, though confirmation is needed for any positive screen to be legally valid. Samples exhibiting a presumptive positive in Screening necessitate additional confirmation.

Should a sample exhibit a presumptive positive during Screening, additional hair is retrieved from the original specimen for Extraction. Drugs are extracted from hair at significantly reduced concentrations compared to alternate methods (such as urine or oral fluid), which contributes to hair drug testing being substantially challenging to conduct.

Confirmation for any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo a washing process before confirmation as required. The complete laboratory progression from Accessioning through Confirmation is evaluated in accordance with both CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
