A 1.5-inch sample of approximately 200 hair strands (akin in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, additional testing, and/or for tests exceeding 10 panels, 150mg is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not available, an equivalent amount of body hair may be used. When we refer to head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial incorporation of a sample into the laboratory's system. This step involves confirming the proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data entry that an electronic chain of custody system did not provide.

Screening entails a preliminary quick assessment for the presence of drugs of abuse. While Screening is an economical means to eliminate drug usage in the majority of samples, a positive result from Screening must be confirmed to be legally valid. All samples that are initially positive in Screening necessitate subsequent confirmation.

If a sample yields a presumptive positive result during the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are drawn from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), rendering hair drug screening as the most intricate methodology.

Confirmation of any positive result from Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples undergo a wash prior to confirmation if necessary. The comprehensive laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
