When collected near the scalp, hair can provide approximately a three-month window for detecting alcohol and other drugs. Hair is easy to obtain, not easily tampered with, and simple to transport.

A sample of around 1.5 inches of about 200 strands of hair (about the size of a #2 pencil) closest to the scalp yields 100mg of hair, the optimal sample for testing and verification. For EtG, additional components, and/or tests exceeding 10 panels, 150mg of the sample is advised. It is suggested to measure the sample using a jeweler’s scale. If scalp hair is inaccessible, an equivalent amount of body hair can be used. When we mention head hair, we mean scalp hair specifically. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a lab’s system. This step includes ensuring the sample is sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening is an initial rapid evaluation for drugs of concern. Although Screening is a cost-efficient method to determine the absence of drug use in most samples, a positive result must be validated to be accepted in court. Any samples that yield a presumptive positive in Screening need further confirmation.

For a sample that is presumptively positive at the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. During this stage, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most complex method to undertake.

Verification of any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples are cleaned prior to verification as required. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
