What it is, How it works

Collecting hair is straightforward, hard to tamper with, and easy to send for testing.

Collecting a 1.5-inch sample from approximately 200 hair strands, roughly the width of a #2 pencil, near the scalp gives 100mg of hair, the optimal quantity for testing. For enhanced EtG detection or tests exceeding 10 panels, a 150mg sample is suggested. We advise using a jeweler’s scale to measure the sample's weight. If scalp hair can't be obtained, an equal quantity of body hair is acceptable. When we mention head hair, it specifically refers to hair from the scalp, while body hair encompasses all other types such as facial or armpit hair.

Process Overview

The drug testing process in the lab includes four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of bringing a sample into the lab's workflow. This involves confirming that the packaging was intact and shipped correctly, designating it a LAN (Laboratory Accessioning Number), and completing any necessary documentation not covered by an electronic chain of custody.

Screening is an initial, rapid examination for substance abuse. While this is a cost-efficient method for eliminating drug use in many samples, a positive result must undergo confirmation for legal acceptability. Samples with potential positive results during Screening require further confirmation.

In cases of potential positive results identified during Screening, additional hair is extracted from the original sample for the Extraction phase. Here, drugs are removed from hair at concentrations lower than other methods (such as in urine or oral samples), making hair drug testing one of the most complex methods.

Confirmed positive screening results are verified using techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, presumptively positive samples are washed if needed. The entire testing procedure, from Accessioning through Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair certification and ISO/IEC 17025 standards compliance.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
