What it is, How it works

Collected near the scalp, hair can reveal up to a three-month detection span for alcohol and drugs. The collection of hair is straightforward, relatively tamper-proof, and easy to send.

A 1.5-inch section consisting of approximately 200 hair strands (about the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal sample size for analysis and confirmation. For EtG, supplementary tests, or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair can be gathered. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The four principal stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial entry of a sample into the laboratory's system. This step includes confirming that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not captured by an electronic chain of custody system.

Screening includes an initial rapid assessment for drugs of abuse. While Screening is an economical method to exclude drug usage on most samples, a positive screen needs confirmation to be valid in legal proceedings. Any samples that are tentatively positive in Screening necessitate a follow-up confirmation.

If a sample is tentatively positive during the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. In this stage, drugs are separated from hair at significantly lower concentrations than in other methods (such as urine or oral fluid), making hair drug screening the most complex method to execute.

Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with tentative positives are cleaned before confirmation as needed. The complete laboratory process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
