What it is, How it works

Hair, collected near the scalp, can reveal a detection window of up to roughly 3 months for alcohol and other substances. A 1.5-inch section of around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample quantity for both screening and confirmation. When testing for EtG, additional tests, and/or panels beyond 10, 150mg of the sample is recommended. It is advised to measure the sample using a jeweler’s scale. If scalp hair is not available, an equal quantity of body hair may be used. Head hair specifically refers to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary stages in lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning handles the initial integration of a sample into a lab's system. It involves checking that the sample was correctly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody system.

Screening comprises a preliminary rapid check for drugs of abuse. Although Screening offers a cost-effective means to eliminate drug use in most samples, a positive screen must be confirmed to be valid in court. Samples that are presumptively positive in Screening necessitate secondary confirmation.

If a sample is presumptively positive in the Screening stage, additional hair is obtained from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at a concentration lower than that used in other methods (e.g., urine or oral fluid), making hair drug screenings the most challenging methodologies to execute.

Confirmation of any positive screening result occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All provisionally positive samples are washed as needed prior to confirmation. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
