A sample around 1.5 inches long containing about 200 strands of hair (roughly equivalent to a #2 pencil) is necessary, offering 100mg of hair, which is ideal for testing. For EtG, additional testing, and screens over 10 panels, 150mg is advised. Use a jeweler’s scale to weigh the hair. If scalp hair isn't available, an equivalent amount of body hair is usable. Scalp hair refers solely to hair from the head. Body hair encompasses other types like facial or axillary hair.

Process Overview

The main stages in processing a drug test in labs include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is about initially handling a sample within a laboratory's system. This involves checking the sample's seal and shipment, assigning a LAN (Laboratory Accessioning Number) randomly, and adding any missing data not available via an electronic chain of custody.

During Screening, an initial quick drug abuse check is performed. Although cost-effective for ruling out drug use in most samples, any positive Screening needs confirmation to be legally acceptable. Presumptive positives in Screening thus require secondary confirmation.

A presumptive positive at the Screening phase prompts more hair to be taken from the original sample for Extraction, where drugs are extracted fainter than from other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug tests.

Confirming any positive result from Screening involves GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptive positive sample is washed prior to confirmation as needed. The laboratory cycle from Accessioning to Confirmation is scrutinized per both CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
