When taken close to the scalp, hair can present roughly a 3-month detection period for alcohol and other drugs. Hair collection is straightforward, challenging to fake, and simple to transport.

A 1.5-inch segment of approximately 200 hair strands (similar in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal sample size for evaluation and verification. For EtG, additional tests, and/or panels above 10, a 150mg specimen is advisable. Using a jeweler’s scale to weigh the sample is encouraged. If scalp hair is not an option, an equivalent amount of body hair can be used. When referring to head hair, we mean scalp hair only; body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four stages integral to processing a drug test result in the lab consist of Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample enters the laboratory system for initial processing. This step verifies the sample was properly sealed and shipped, assigns a LAN (Laboratory Accessioning Number), and completes any missing data entry not covered by an electronic chain of custody system.

Screening conducts a preliminary assessment for drug abuse. While it is a cost-efficient method to dismiss drug usage in most samples, any positive screen must be confirmed for legal acceptance. Presumptively positive samples during Screening need a follow-up confirmation.

For samples presumed positive in the Screening stage, additional hair is extracted from the original specimen and prepped for Extraction. Here, drugs are retrieved from hair at concentrations much lower than alternative methods (e.g., urine or oral fluid), emphasizing the challenge of hair drug screening as a methodology.

Any positive findings from screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are washed as needed. The entire process, from Accessioning to Confirmation, undergoes review under the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
