For EtG, additional tests, or panels beyond 10, a 150mg specimen is advised. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is unavailable, a similar amount of body hair may be used. Mention of head hair refers only to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processes a drug test result through four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step in processing a sample within a lab’s system, which includes checking the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and inputting any extra data not provided by electronic chain of custody systems.

Screening offers an initial rapid assessment for abuse drugs. While a cost-efficient method for ruling out drug use in most samples, a positive Screening result necessitates Confirmation to be valid in legal settings. Any samples presumed positive in Screening require a second confirmation.

In cases of a presumptive positive outcome in Screening, more hair is sourced from the original specimen and prepared for Extraction, where drugs are isolated at much lower concentrations compared to other methods (such as urine or oral fluids), making hair drug screening more complex.

Any positive Screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo necessary washing before confirmation. The entire lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
