What it is, How it works

Hair analysis has emerged as an effective approach for uncovering drug and alcohol consumption. Hair captures a long-term record of drugs and alcohol by incorporating biomarkers into its fibers as it grows. When collected near the scalp, hair can reveal up to approximately a 3-month detection window for alcohol and other substances. Collecting hair is straightforward, not easy to tamper with, and straightforward to transport.

A 1.5-inch segment consisting of about 200 hair strands (similar to the thickness of a #2 pencil) from the scalp will provide 100mg of hair, considered the optimal quantity for both screening and confirmation processes. For EtG, supplementary tests, and/or analyses exceeding 10 panels, 150mg of the sample is advised. It's suggested to measure the sample’s weight using a jeweler’s scale. In the absence of scalp hair, a comparable quantity of body hair can be acquired. When referring to head hair, it specifically pertains to scalp hair. Body hair includes all other types of hair (such as facial, axillary, etc.).

Process Overview

There are four primary steps in the laboratory handling of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the sample’s initial processing into the laboratory’s system. This phase entails ensuring the sample’s proper sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and executing any further data entry not already addressed by an electronic chain of custody protocol.

Screening involves a preliminary, rapid inspection for drug misuse. Although Screening is an economical method to exclude drug consumption in most specimens, any sample indicating a positive screen must undergo Confirmation to be legally acceptable. Samples identified as presumptively positive during Screening necessitate a subsequent confirmation.

If a specimen is presumptively positive during the Screening stage, additional hair is extracted from the original sample and readied for Extraction. In this phase, substances are retrieved from hair at a lower concentration compared to other methods (such as urine or oral fluid), which makes hair drug analysis the most challenging method to execute.

Confirmation of any presumptive positive result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before Confirmation when required. The entire lab procedure from Accessioning through Confirmation undergoes review according to CAP (College of American Pathologists) Hair designation and accreditation under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
