"Body hair" encompasses all other hair types (e.g., facial, axillary, etc.).

Process Overview

The main procedural steps in the laboratory examination of a drug test result encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample within a laboratory's framework. This step verifies the sample's proper sealing and shipment, assigns it a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry absent from an electronic chain of custody system.

Screening is a preliminary assessment to detect potential drugs of abuse. Although cost-efficient and effective for the majority of samples, a positive screen necessitates confirmation to be considered legally valid. Those samples showing preliminary positive results indeed require additional confirmation.

Should a sample yield a presumptive positive at the Screening stage, more hair is extracted from the initial collection for the Extraction process. Here, drugs are isolated from the hair at markedly reduced concentrations compared to other methodologies (such as urine or oral fluid), positioning hair drug testing as one of the more challenging methods to perform.

Confirmatory analysis of any positive screening is executed using techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptive positives undergo washing before confirmation if necessary. The entire laboratory workflow from Accessioning through Confirmation is evaluated under both the CAP (College of American Pathologists) Hair guidelines and accredited against ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
