A 1.5-inch section comprising around 200 hair strands (approximately the thickness of a #2 pencil) near the scalp produces 100mg, which is the preferred amount for both screening and confirmation. For EtG tests, additional tests, and/or panels above 10, a sample size of 150mg is suggested. Using a jeweler’s scale to weigh the sample is recommended. If no scalp hair is available, a similar quantity of body hair is an alternative. "Head hair" refers exclusively to scalp hair, whereas "body hair" encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory process for drug test analysis includes four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial phase of sample integration into the lab's system. This step involves confirming proper sealing and shipping of the sample, generating a random LAN (Laboratory Accessioning Number), and finalizing any additional data input absent from an electronic chain of custody system.

Screening performs a preliminary and rapid evaluation for substances of abuse. Screening offers a budget-friendly means to eliminate drug usage possibilities for most samples; however, a positive screen necessitates further confirmation for courtroom applicability. Samples testing presumptively positive during Screening will need subsequent confirmation.

In cases where Screening indicates a presumptive positive, additional hair is drawn from the initial supply for the Extraction phase. This phase isolates drugs from hair at considerably lower concentrations than other methods (e.g., urine, oral fluid), which explains the complexities of conducting hair drug screening.

Verification of any positive screening outcomes utilizes techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptive positive sample is cleansed before confirmation as required. The entire process from Accessioning through Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
