For EtG tests, extended panels, or screening beyond 10 panels, a mass of 150mg is suggested. Utilizing a jeweler’s scale for accuracy in measuring the sample is advisable. In cases lacking scalp hair, equivalent quantities of body hair may be used. Our reference to head hair pertains exclusively to scalp hair, while body hair encompasses all other types (e.g., facial, axillary).

Process Overview

Lab processing for drug test analysis incorporates four essential stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of the sample into the laboratory’s framework. This step includes confirmation of proper sealing and shipment, assignment of a random LAN (Laboratory Accessioning Number), and completion of any data entry absent from an electronic chain of custody system.

Screening involves a preliminary assessment for substance abuse. Despite its cost-effectiveness in eliminating negative samples, a positive result in Screening requires further confirmation for legal validity. Samples with presumptive positives in Screening necessitate verification.

In instances of presumptive positives found during Screening, additional hair is extracted from the original specimen for the Extraction phase. Here, drug compounds are isolated from the hair at concentrations lower than those observed with alternate methods (e.g., urine or oral fluid), underscoring hair drug analysis as a complex process.

Confirmed positive results are verified using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with preliminary positive findings undergo washing before Confirmation if necessary. The comprehensive laboratory work from Accessioning to Confirmation adheres to CAP (College of American Pathologists) Hair designation as well as ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
