For EtG, additional assessments, and/or analyses beyond 10 panels, 150mg of the sample is advisable. It's suggested to use a jeweler’s scale for specimen measurement. In the absence of scalp hair, an equivalent volume of body hair may be gathered. The term head hair specifically refers to scalp hair. Body hair encompasses other hair types (such as facial and axillary hair).

Process Overview

The laboratory processing of a drug test result involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a specimen into the laboratory’s system. This process verifies that the sample was properly sealed and shipped, allocates a random LAN (Laboratory Accessioning Number), and completes any supplementary data input not addressed by an electronic chain of custody system.

Screening encompasses a preliminary assessment for abused substances. Although Screening provides an economical method to exclude drug use in most specimens, screens returning positive results require further confirmation to be legally acceptable. Samples that display presumptive positives during Screening necessitate additional confirmation.

When a sample shows a presumptive positive in Screening, an extra portion of hair is extracted from the original specimen for preparation in the Extraction phase. In Extraction, drugs are isolated from hair at lower concentrations than other methodologies (e.g., urine or oral fluid), rendering hair drug testing particularly challenging to conduct.

Confirmation of any positive screen is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, samples that are presumptively positive are washed as necessary. The complete laboratory workflow, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
