What it is, How it works

The utilization of hair testing has emerged as an effective method for identifying alcohol and drug usage. Hair offers an extended timeline of drug and alcohol presence, as it secures biomarkers within the growing hair strand's fibers. When strands are collected near the scalp, they offer an approximate 3-month period for detecting alcohol and drug presence. Hair collection is straightforward, challenging to tamper, and easy to transport.

Collecting a 1.5-inch sample of roughly 200 hair strands (approximately the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, the perfect amount for screening and confirmation purposes. For tests involving EtG, additional screening, or those exceeding 10 panels, a 150mg specimen is advised. It is beneficial to weigh the specimen using a jeweler’s scale. In the absence of scalp hair, a comparable amount of body hair is acceptable. References to head hair specifically mean scalp hair. Body hair pertains to all other types (including facial and axillary).

Process Overview

The procedure for processing a drug test in the laboratory encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the primary processing of a sample within the lab's system. This involves confirming that the sample is properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening involves an initial rapid assessment for drug abuse. While Screening is an economical approach to exclude drug use for most samples, a positive screen must be confirmed for legal admissibility. Samples deemed presumptively positive during Screening necessitate secondary confirmation.

For presumptively positive samples in the Screening phase, additional hair is extracted from the primary specimen and prepared for Extraction. Here, drugs are extracted from hair in concentrations significantly lower than other methods (e.g., urine or oral fluid), contributing to the complexity of hair drug screening.

Confirmation of any positive screening result occurs through GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are cleansed prior to confirmation if needed. The full laboratory process from Accessioning through Confirmation is overseen under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditations.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
