A 1.5-inch sample, comprised of roughly 200 hair strands (approximately the thickness of a #2 pencil) taken close to the scalp will yield 100mg of hair, which is the optimal amount for both screening and confirmation purposes. For EtG, additional testing, and tests beyond 10 panels, a sample of 150mg is suggested. We advise using a jeweler’s scale for weighing the sample. In the absence of scalp hair, an equivalent amount of body hair can be used. References to head hair pertain solely to scalp hair, whereas body hair refers to all other types such as facial hair and axillary hair.

Process Overview

The laboratory procedure for processing a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is registered into the laboratory system. This stage confirms the sample's seal and proper shipment, assigns it a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry that an electronic chain of custody system does not cover.

Screening serves as a preliminary inspection for substances of abuse. This stage is an economical method for detecting substance use in most samples, but a positive result requires confirmation for legal admissibility. Any samples that test positive in the Screening phase require further verification.

Should a sample test positively in the Screening phase, more hair is extracted from the initial specimen to proceed with Extraction. Here, the concentration of drugs extracted from hair is significantly lower than that from other methods (e.g., urine or oral fluid), thus making hair drug testing notably challenging.

Verification of any positive Screening results is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples preliminarily labeled as positive undergo washing prior to confirmation as warranted. The entire laboratory sequence from Accessioning through Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and aligned with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
