What it is, How it works

When clipped close to the head, hair can provide a detection period of up to about three months for alcohol and other drugs. Hair is easy to collect, somewhat challenging to tamper with, and straightforward to send.

A sample of hair measuring 1.5 inches with nearly 200 strands (comparable to the thickness of a #2 pencil) near the scalp will yield 100mg, which is the perfect sample for evaluation and confirmation. For EtG, additional testing, and/or tests surpassing 10 panels, a 150mg sample is suggested. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is not available, an equivalent quantity of body hair can be collected. When mentioning head hair, we specifically mean scalp hair. Body hair refers to all other hair types (facial, axillary, etc.).

Process Overview

The four key phases in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial insertion of a sample into a lab’s system. This process verifies that the sample was sealed and dispatched correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any remaining data entry not covered by an electronic chain of custody system.

Screening entails a preliminary quick check for substances of abuse. Although Screening is a cost-effective approach to eliminating drug consumption in most samples, a positive screen requires confirmation to be valid in court. Any samples that test presumptively positive in Screening necessitate a secondary affirmation.

If a specimen tests presumptively positive during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. This phase involves extracting drugs from hair at a substantially lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug analysis is the most challenging methodology to execute.

Confirmation of any positive screen result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned prior to confirmation as necessary. The complete lab process from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
