When taken close to the scalp, hair can offer a detection period of up to about 3 months for alcohol and drugs. Hair is straightforward to gather, relatively hard to tamper with, and simple to dispatch.

A section measuring 1.5 inches and comprising roughly 200 hair strands (around the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the perfect sample for analysis and verification. For EtG additional tests and/or screenings over 10 panels, 150mg of the sample is advised. It is suggested to measure the sample on a jeweler's scale. If scalp hair is not available, a similar quantity of body hair may be used. Head hair pertains solely to scalp hair. Body hair denotes all other hair categories (facial, axillary, etc.).

Process Summary

The primary stages in the laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary handling of a specimen into a lab's framework. This entails confirming that the sample was properly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent in an electronic chain of custody framework.

Screening involves an initial rapid assessment for drugs of abuse. Though Screening is a more budget-friendly means of eliminating drug usage for the majority of samples, a positive screen must be confirmed for legal admissibility. Any samples marked as presumptively positive in Screening necessitate a secondary confirmation.

In instances where a sample is presumptively positive during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), which is why hair drug testing is considered the most challenging procedure to execute.

Verification of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed prior to confirmation if required. The entire lab procedure from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
