Should scalp hair be unavailable, an equivalent quantity of body hair can be harvested. When scalp hair is referenced, it specifically pertains to hair from the head's top. Conversely, body hair encompasses all other hair types, such as facial and axillary hair.

Process Overview

There are four primary phases in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a specimen into the lab’s framework. This involves ensuring the sample was correctly sealed and dispatched, assigning an arbitrary LAN (Laboratory Accessioning Number), and filling in any additional data not incorporated via an electronic chain of custody system.

Screening is a preliminary rapid test for illicit substances. Although Screening is an economical method to dismiss drug presence in most samples, a positive result in Screening mandates confirmation to be permissible in legal proceedings. Samples with a presumptive positive result during Screening necessitate subsequent confirmation.

Upon a presumptive positive outcome in Screening, supplementary hair is extracted from the initial sample to prepare it for Extraction. Here, the drugs are isolated from hair at a noticeably lower concentration compared to other techniques, such as those for urine or oral fluids, underscoring the intricate nature of hair drug screening.

Confirmation of any positive Screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undergo washing before confirmation as specified. The entirety of the laboratory process, from Accessioning to Confirmation, is assessed in accordance with both the CAP (College of American Pathologists) Hair designation and adherence to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
