For EtG, additional tests, or those exceeding 10 panels, a 150mg specimen is advisable. It's suggested to use a jeweler’s scale to weigh the sample. If scalp hair is not available, a similar quantity of body hair may be gathered. When discussing head hair, we refer solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary phases in processing a drug test in the laboratory include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a lab’s system. This process verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data processing not captured by the electronic chain of custody.

Screening is an initial rapid evaluation for drugs of abuse. While Screening is a cost-efficient method to eliminate drug presence on most samples, a positive screen requires confirmation to be legally valid. Presumptive positive samples in Screening necessitate further confirmation.

Should a sample test positive in the Screening phase, additional hair is extracted from the original specimen for the Extraction phase. At this stage, drugs are removed from hair at much lower concentrations than with other methods (e.g., urine or oral fluid), which is why hair drug screening is notably challenging to conduct.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positives are cleaned prior to confirmation as needed. The complete lab process from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
