What it is, How it works

Hair testing is recognized as a robust method for identifying drug and alcohol consumption. Hair offers an extended historical record of alcohol and other drugs by encasing biomarkers within the strands during growth. When sourced close to the scalp, hair can offer up to an approximate 3-month detection period for alcohol and various substances. Hair collection is straightforward, relatively tamper-proof, and simple to transport.

A 1.5-inch sample comprising about 200 hair strands (approximating the thickness of a #2 pencil) nearest to the scalp yields 100mg, which is the preferred quantity for analysis and verification. For EtG, additional tests, or panels exceeding 10, 150mg is advisable. Using a jeweler’s scale to weigh the sample is suggested. Should scalp hair be unavailable, a similar quantity of body hair can be used. It's important to note that 'head hair' pertains exclusively to scalp hair, while 'body hair' encompasses all other hair types (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in a laboratory consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase, where a sample is entered into the lab’s system. This involves confirming the sample’s proper sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and completing any data entry omitted by an electronic chain of custody protocol.

Screening offers a preliminary assessment for substances of abuse. Although Screening is an economical means to rule out drug use in most cases, positively screened samples need confirmation for legal admissibility. Any samples preliminarily positive in Screening require additional validation.

In cases of preliminary positive results during Screening, additional hair is extracted from the initial specimen and prepared for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than those in other testing methods (e.g., urine or oral fluid), rendering hair drug screening the most challenging method to execute.

Confirmation of positive screening outcomes is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with assumed positive results undergo washing prior to confirmation as necessary. The complete laboratory sequence from Accessioning to Confirmation is reviewed in accordance with both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
