Hair testing is increasingly recognized as a robust method for identifying drug and alcohol use. By capturing biomarkers within the fibers of a developing hair strand, it offers a prolonged history of alcohol and other substances. When clipped near the scalp, hair can reveal drug and alcohol consumption for up to three months. Hair is easy to take, a challenge to tamper with, and is simple to send.
A 1.5-inch section of about 200 hair strands (comparable to a #2 pencil's width) closest to the scalp will yield 100mg of hair, the preferred sample amount for evaluations and confirmations. For EtG, supplementary tests, and/or screens exceeding 10 panels, 150mg of the specimen is advised. It's best to measure the specimen's weight with a jeweler's scale. If scalp hair isn't accessible, an equivalent quantity of bodily hair may be utilized. When we mention head hair, we mean only scalp hair. Body hair encompasses all other hair categories (such as facial, axillary, etc.).
Process Overview
There are four primary stages in a laboratory's analysis of drug testing: Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the initial processing step where a sample is integrated into a lab's framework. This step includes verifying proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional details absent from an electronic chain of custody.
Screening is a preliminary check for substances of abuse. Though efficient in ruling out drug usage in most samples, a positive outcome requires validation for legal proceedings. Any samples testing presumptively positive at this stage need secondary confirmation.
If the Screening phase suggests a positive result, additional hair is sourced from the original sample for Extraction. This phase involves extracting drugs from the hair, where the substance concentration is considerably lower than other methods (e.g. urine or oral fluid), making hair drug screening particularly challenging.
Any positive screening outcomes undergo Confirmation via GC/MS, GC/MS/MS or LC/MS/MS methods. Presumptive positive samples are cleansed as needed before confirmation. From Accessioning to Confirmation, the entire process is guided by the CAP (College of American Pathologists) Hair qualification and accredited under ISO / IEC 17025 standards.

Limitations:
Note: Though often termed "hair follicle tests", the examination analyzes the hair strand itself, not the follicle beneath the scalp.
