Should scalp hair be inadequate, a similar portion of body hair may be taken. When mentioning head hair, it pertains solely to scalp hair. Body hair includes all other hair categories (facial, axillary, etc.).

Process Overview

The four primary procedures in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the preliminary handling of a sample into a laboratory's framework. This involves verifying the sample’s securement and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry absent from an electronic chain of custody system.

Screening encompasses a preliminary, quick evaluation for drugs of abuse. Although Screening is a cost-efficient means to discount drug use on most samples, any screen yielding a positive outcome must be verified to be accepted in legal settings. Samples that show potential positives in Screening must undergo a follow-up confirmation.

Should a sample exhibit a presumptive positive during Screening, additional hair is sourced from the original sample and prepped for Extraction. In this phase, drugs are liberated from hair at concentrations much lower than other techniques (ex. urine or oral fluid), making hair drug screening an exceptionally complex technique to execute.

Confirmation of any positive screen is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair classification and the accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
