What it is, How it works

Utilizing hair testing has emerged as a potent method for identifying drug and alcohol consumption. Hair preserves a prolonged record of substance use by embedding biomarkers within the hair strand fibers as they grow. When taken close to the scalp, hair offers a detectability period extending up to roughly three months for alcohol and drug presence. Hair collection is straightforward, comparatively hard to tamper with, and convenient to send.

A specimen of 1.5 inches, consisting of around 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp, yields 100mg of hair, the optimal quantity for evaluation and verification. For EtG, additional tests, and/or panels exceeding ten, a specimen amount of 150mg is advised. It's suggested to measure the sample weight utilizing a jeweler’s scale. Should scalp hair be absent, an equivalent volume of body hair is permissible for collection. The term "head hair" specifically refers to scalp hair, while "body hair" encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory procedures for processing drug tests encompass four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning relates to the preliminary integration of a sample into the laboratory system. Tasks include ensuring the sample is appropriately sealed and transported, assigning a randomized LAN (Laboratory Accessioning Number), and finalizing any supplemental data entry absent from an electronic custody tracking system.

Screening represents an initial rapid assessment for substance misuse. While Screening is a cost-effective approach to exclude drug usage in most samples, any positive detection requires verification for judicial validity. Samples that initially test positive during Screening mandate further Confirmation.

Should a sample indicate a presumptive positive in the Screening phase, additional hair is separated for the Extraction step. During Extraction, drugs are isolated from hair at considerably less concentration compared to other techniques (e.g., urine or oral fluid), underpinning the complexity of conducting hair drug screening.

Confirmation of positive Screening results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. Presumptive positive samples undergo washing prior to Confirmation as necessitated. The entire laboratory process, from Accessioning through Confirmation, undergoes scrutiny under both the CAP (College of American Pathologists) Hair label and ISO/IEC 17025 standard accreditation.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
