A 1.5-inch portion, approximately 200 hair strands (comparable to a #2 pencil in thickness) nearest to the scalp, yields 100mg of hair, which is the preferred sample size for analysis and confirmation. For EtG, add-ons, and/or assessments beyond 10 panels, 150mg of the sample is advised. The use of a jeweler’s scale is suggested for measuring the specimen. If scalp hair is unavailable, an equivalent quantity of body hair can be collected. References to head hair pertain solely to scalp hair, whereas body hair includes all other varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is divided into four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing phase, integrating the sample into the laboratory’s system. It involves confirming that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry tasks not covered by an electronic chain of custody system.

Screening is a preliminary examination to identify potential drug use. While Screening is an economical method to rule out drug presence in most samples, a positive screen requires confirmation to be legally permissible. Any samples that yield a presumptive positive outcome in Screening necessitate secondary verification.

In case a sample yields a presumptive positive result during Screening, additional hair is extracted from the original sample and prepared for Extraction. At this stage, drugs are extracted from hair at a much lesser concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Verification of any positive screen result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are washed if necessary. The entire laboratory process, from Accessioning through Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
