Process Overview

There are four key phases in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample within a lab's system. This step ensures the sample was properly sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entry that an electronic chain of custody system may not furnish.

Screening conducts a preliminary rapid assessment for substances of abuse. Although Screening is a budget-friendly method to exclude drug use in the bulk of samples, a positive screening result must be confirmed to stand in legal proceedings. Any samples with a provisional positive indication in Screening demand follow-up confirmation.

If a sample shows a tentative positive result in the Screening phase, further hair is taken from the original sample for Preparation in Extraction. In this phase, drugs are retrieved from hair at a considerably lower concentration compared to alternate methods (e.g., urine or oral fluid), rendering hair drug screening the most complex avenue to undertake.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Every tentative positive sample is cleansed before confirmation, as per requirement. The laboratory process, encompassing Accessioning through Confirmation, is examined under both the CAP (College of American Pathologists) Hair classification and accreditation to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
