Hair analysis is now regarded as an effective means for identifying drug and alcohol consumption. Hair offers an extended timeline of alcohol and other substance usage by capturing biomarkers within the strands of hair. When taken near the scalp, hair can show up to a 3-month detection window for both alcohol and drugs. Hair collection is straightforward, less prone to tampering, and easily transportable.

A sample of 1.5 inches consisting of roughly 200 hair strands (comparable to a #2 pencil) nearest the scalp provides 100mg of hair, which is optimal for testing and validation. For EtG, additional tests, and/or screens beyond 10 panels, a sample of 150mg is advisable. It is recommended to measure the sample on a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair may be sourced. Head hair refers strictly to scalp hair. Body hair encompasses all other types such as facial, axillary, etc.

Process Overview

The principal steps in the lab processing of a drug screen are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary integration of a sample into a lab's system. This involves checking that the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening conducts a preliminary rapid check for substances of abuse. Should a sample be presumptively positive during Screening, additional hair is extracted from the original sample and prepped for Extraction. In this phase, drugs are extracted from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug testing the most challenging method to execute.

Any positive screen results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positives are washed as warranted. The complete lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair category and ISO / IEC 17025 accreditation standards.
