What it is, How it works

For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. It is suggested to weigh the sample using a jeweler’s scale. If head hair is unavailable, an equivalent quantity of body hair can be used. When discussing head hair, we mean scalp hair exclusively. Body hair comprises all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four critical steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory's system. This process includes verifying proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry that an electronic chain of custody system does not provide.

Screening involves a preliminary inspection for drug abuse. Although it is a cost-effective measure to eliminate drug usage in most cases, a positive Screening requires confirmation for legal admissibility. Samples that test presumptively positive necessitate secondary confirmation.

If a sample tests presumptively positive in Screening, additional hair is extracted from the original sample for Extraction. In this stage, drugs are separated at significantly lower concentrations from hair compared to other methods (e.g., urine or oral fluid), which is what makes hair drug screening the most challenging methodology to execute.

To confirm any positive screening results, GC/MS, GC/MS/MS, or LC/MS/MS is employed. Presumptive positives are washed as necessary before confirmation. The full process, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
