If scalp hair is not accessible, a similar quantity of body hair may be used. It is important to note that head hair specifically means scalp hair, while body hair encompasses all other types of hair (such as facial and axillary).

Process Overview

The laboratory processing of a drug test follows four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step, where a sample is introduced into a lab’s system. This phase includes confirming proper sealing and shipment of the sample, generating a unique LAN (Laboratory Accessioning Number), and processing any additional necessary data if an electronic chain of custody system doesn’t provide it.

Screening provides a preliminary assessment for drug presence. Though cost-effective in excluding drug use in many samples, a positive screen necessitates confirmation for legal proceedings. Samples that are initially positive in Screening must undergo further confirmation.

A sample deemed positive in the Screening requires additional hair from the primary specimen for Extraction. In this phase, drugs are extracted from the hair at much lower levels than methods like urine or oral fluid, making hair drug screening notably challenging.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples with presumed positive outcomes are cleaned before Confirmation as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
