A 1.5-inch piece of around 200 strands of hair (approximately the thickness of a #2 pencil) nearest the scalp will secure 100mg of hair, the optimum quantity for both initial screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is suggested. We propose using a jeweler’s scale to measure the specimen. If head hair cannot be sourced, a matching amount of body hair could be used. By head hair, we mean only hair at the scalp. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory stages for processing a drug test outcome consist of Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample is initially processed into a laboratory's system. This involves confirming the sample was properly sealed and shipped, assigning it a random LAN (Laboratory Accessioning Number), and completing any extra data input absent from an electronic chain of custody system.

Screening involves an initial check to identify drugs of abuse. Though Screening is a more economical way to exclude drug usage in most samples, a positive result from screening requires confirmation to be valid in legal proceedings. Samples that are tentatively positive during Screening necessitate secondary confirmation.

If a sample shows a tentative positive result in the Screening phase, additional hair is taken from the initial sample and prepared for Extraction. At this phase, drugs are extracted from the hair at significantly lower concentrations than with other methods (such as urine or oral fluid), making hair drug testing the most challenging method to execute.

The Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples tentatively deemed positive are cleaned before confirmation if necessary. The complete lab process from Accessioning through to Confirmation is assessed following both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 certification standards.
