What it is, How it works

Hair analysis has emerged as a potent method for identifying substance use. Hair captures a lengthy history of alcohol and drug consumption by embedding biomarkers within the fibers of the hair as it grows. Collecting hair close to the scalp can give a detection span of up to around three months for alcohol and drugs. The collection of hair is straightforward, making it somewhat resistant to tampering, and effortless to transport.

A 1.5-inch section of roughly 200 hair strands (similar to a #2 pencil in size) closest to the scalp yields 100mg of hair, the preferred amount for screening and validation. For EtG, additional tests, or tests over 10 panels, 150mg of the hair is advised. Measuring the specimen with a jeweler’s scale is recommended. In the absence of scalp hair, an equivalent amount of body hair may be gathered. When mentioning head hair, we mean scalp hair solely. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The principal steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into the lab's system. This comprises confirming that the sample was sealed and dispatched correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry not available via an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening is an economical method to exclude drug use for the bulk of samples, a positive screen necessitates confirmation to be valid in legal proceedings. Any samples that are presumptive positives in Screening require secondary confirmation.

If a sample appears positive in the Screening phase, more hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are withdrawn from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), which is why hair drug screening is the hardest method to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are thoroughly washed before confirmation as necessary. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
