A 1.5-inch segment of around 200 hair strands (approximately the diameter of a #2 pencil) closest to the scalp provides 100mg, which is the optimal sample for assessment and verification. For EtG, add-ons, and/or tests involving more than 10 panels, 150mg of the sample is suggested. We advise measuring the sample with a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be gathered. When mentioning head hair, we refer to scalp hair solely. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample into a lab’s system. This step includes ensuring the sample’s seal and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody system.

Screening entails a preliminary rapid assessment for substances of abuse. Though Screening is a cost-effective method to exclude drug usage in most samples, a positive screen must be confirmed to be valid in legal proceedings. Any samples that appear positive during Screening necessitate a further confirmation step.

If a sample appears positive during the Screening phase, additional hair is extracted from the initial sample and made ready for Extraction. In this phase, drugs are retrieved from hair at a considerably lower concentration than other methods (e.g., urine or oral fluid), which contributes to the complexity of hair drug testing.

Confirmation of any positive outcome from Screening is done utilizing GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumed positive results are cleansed before confirmation as required. The complete lab process from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
