What it is, How it works

Gathered near the scalp, hair can reflect a detection span of up to about three months for alcohol and drugs. The collection process is straightforward, the possibility of tampering is relatively low, and it is easily transportable.

A sample measuring 1.5 inches consisting of roughly 200 hair strands (approximating the girth of a #2 pencil) taken closest to the scalp provides 100mg of hair, which is optimal for both screening and verification. For tests involving EtG, additional add-ons, and/or panels exceeding 10, a specimen amounting to 150mg is advised. It's best to use a jeweler’s scale for determining the specimen's weight. Should scalp hair not be present, an equivalent quantity of body hair can be collected. By head hair, we mean exclusively scalp hair. Body hair encompasses all other types including facial, axillary, etc.

Overview of the Process

There are four primary stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the lab's system. This process includes confirming that the sample was appropriately sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and fulfilling any additional data input not covered by an electronic chain of custody protocol.

The Screening phase involves a preliminary rapid assessment for drugs of concern. While Screening is an economical approach to eliminate the majority of samples potentially indicating drug use, a positive screen requires confirmation for legal admissibility. Samples that test positive default in Screening necessitate a subsequent verification.

Should a sample exhibit a presumptive positive result in Screening, additional hair is extracted from the original specimen for preparation during the Extraction stage. In this phase, drugs are isolated from hair at much lower concentrations compared to other techniques (such as urine or saliva), establishing hair drug screening as the most challenging methodology to execute.

Confirmation for any positive screen outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating presumptive positives are cleansed before undergoing confirmation as needed. Each stage of the lab process from Accessioning to Confirmation is assessed according to both the CAP (College of American Pathologists) Hair categorization and the accreditation by ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
