What it is, How it works

The utilization of hair testing is widely recognized as an effective method for identifying drug and alcohol consumption. When hair is taken close to the scalp, it can reveal up to about a 90-day detection period for alcohol and drugs. Hair is easy to collect, less likely to be tampered with, and convenient to transport.

A hair sample measuring 1.5 inches and comprising roughly 200 hairs (equivalent to a #2 pencil's thickness) nearest to the scalp will yield 100mg of hair, which is the perfect quantity for both screening and confirmation. For EtG, add-ons, and tests exceeding 10 panels, a 150mg sample is preferred. Using a jeweler's scale to weigh the sample is advisable. In the absence of scalp hair, a similar amount of body hair can be used instead. The term head hair specifically refers to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The processing of a drug test result in the lab involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory's system. This process includes checking that the sample is properly sealed and transported, assigning it a unique LAN (Laboratory Accessioning Number), and filling in any extra data missing from an electronic chain of custody system.

Screening is a preliminary test for drugs of abuse. This cost-effective measure is useful for eliminating drug use in most samples; however, positive screens must be confirmed to be accepted in legal settings. Samples that are presumptively positive in Screening require a further confirmation step.

If a sample is presumptively positive following Screening, additional hair is extracted from the original sample for the Extraction stage. Drugs are extracted from hair at much lower concentrations here than in alternative methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive screening outcomes is carried out using GC/MS, GC/MS/MS or LC/MS/MS. Samples that return a presumptive positive are washed before confirmation, as necessary. From Accessioning through to Confirmation, the entire process is reviewed under both CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
