What it is, How it works

When taken from near the scalp, hair enables a detection period reaching up to around 3 months for alcohol and drugs. Collection is straightforward, tampering is somewhat challenging, and transportation is uncomplicated.

A sample size of 1.5 inches, consisting of roughly 200 hair strands (equivalent to a #2 pencil), closest to the scalp yields 100mg, which is the optimal quantity for both screening and validation. For EtG, supplementary tests, and/or screenings exceeding 10 panels, a specimen of 150mg is advisable. Using a jeweler’s scale for weighing the specimen is suggested. If scalp hair is inaccessible, a comparable amount of body hair can be utilized. "Head hair" specifically denotes scalp hair. "Body hair" encompasses all other hair types (e.g., facial, axillary, etc.).

Process Overview

There are four principal stages in processing a drug test result in a lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of incorporating a sample into a lab’s system. It entails ensuring the sample’s seal and proper shipment, allocating a random LAN (Laboratory Accessioning Number), and handling any remaining data entry not covered by an electronic chain of custody system.

Screening serves as an early assessment for substance abuse. Although Screening efficiently eliminates drug use in most cases, a positive screen necessitates confirmation for court approval. Presumptive positive samples in Screening do call for a subsequent confirmation.

Should a sample be presumptively positive during Screening, more hair is extracted from the original sample for the Extraction phase. During this phase, substances are removed from hair at a significantly lower concentration relative to other methods (e.g., urine or oral fluid), which makes hair drug screening particularly complex to execute.

Confirmation of any presumptive positive result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptive positive sample is cleaned before confirmation if necessary. The entire lab process from Accessioning through Confirmation undergoes scrutiny under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
