What it is, How it works

A 1.5-inch segment of approximately 200 hair strands (around the thickness of a #2 pencil) closest to the scalp supplies 100mg of hair, which is the optimal sample size for both screening and verification. For EtG requests, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to measure the specimen is suggested. If scalp hair cannot be obtained, an equivalent volume of body hair can be used. In terms of head hair, we're specifically referring to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result involves four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initiation of processing a specimen into a laboratory's records. This step ensures the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and fills in any supplementary data entry missing from an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. Screening offers a cost-effective method to exclude drug usage for most samples; however, to be valid in a legal setting, a positive screening must be verified. Any samples that test as presumptively positive during Screening demand a follow-up confirmation.

If a sample tests as presumptively positive during the Screening phase, additional hair is extracted from the original sample in preparation for Extraction. This phase extracts drugs from hair at a lower concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to perform.

Verification of any presumptively positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive findings undergo necessary washing before confirmation. The complete lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
