What it is, How it works

It offers a comprehensive view of alcohol and other drug use by encasing biomarkers within the fibers of the hair strand as it grows. When taken from near the scalp, hair can offer a detection timeframe of up to about 90 days for alcohol and other drugs. Hair collection is straightforward, somewhat resistant to tampering, and easy to transport.

A 1.5-inch section of approximately 200 hair strands (roughly the width of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the ideal amount for assessment and verification. For EtG, additional tests, and/or those exceeding 10 panels, a 150mg sample is suggested. It's advised to weigh the specimen using a jeweler's scale. If scalp hair is not available, an equivalent amount of body hair can be gathered. When mentioning head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary stages involved in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's system. This process includes verifying the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not covered by an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient method for eliminating drug usage in most samples, a positive screen must be verified to be legally acceptable. Any samples that are indicative of a positive result in Screening do necessitate a secondary confirmation.

If a sample is indicative of a positive result during the Screening stage, additional hair is extracted from the initial specimen and prepped for Extraction. During this phase, drugs are extracted from hair at much lower concentrations compared to other techniques (e.g., urine or oral fluid), which is why hair drug screening ranks as the most challenging methodology.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All indicative positive samples are washed before confirmation if needed. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
