A 1.5-inch segment consisting of about 200 hair strands (approximately the thickness of a #2 pencil) near the scalp yields 100mg, which is the optimal amount for analysis. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of hair is advised. Using a jeweler’s scale for weighing the sample is suggested. If scalp hair cannot be obtained, an equivalent quantity of body hair may be used. 'Head hair' refers exclusively to scalp hair, whereas 'body hair' includes all other hair types, such as facial or axillary hair.

Process Overview

The laboratory analysis of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning addresses the initial handling of a sample within a laboratory’s framework. This step ensures the sample was appropriately sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not covered by an electronic chain of custody system.

Screening performs an initial rapid assessment for drugs of abuse. Though Screening is economical for excluding drug use in most samples, positive results require confirmation for legal admissibility. Samples with presumptively positive results in Screening necessitate a secondary confirmation.

For samples presumed positive in Screening, more hair is extracted from the initial specimen for the Extraction phase. In this phase, drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening methodologies.

Confirmation of positive screening results employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives undergo washing before confirmation as required. The entire laboratory process from Accessioning to Confirmation is executed under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 accreditation benchmarks.
