What it is, How it works

Hair analysis is increasingly recognized as an effective method for detecting drug and alcohol consumption. Hair serves as a lasting record of alcohol and drug intake by capturing biomarkers within the strands' fibers. When trimmed near the scalp, hair can reveal drug and alcohol use over nearly a 3-month period. Hair collection is straightforward, challenging to tamper with, and simple to transport.

A 1.5-inch section of roughly 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp yields about 100mg of hair, the optimal sample for assessment and validation. For EtG, additional panels, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's advisable to weigh the sample using a jeweler’s scale. If scalp hair is missing, an equivalent quantity of body hair may be gathered. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other hair variants (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning concerns the initial integration of a sample into a laboratory’s framework. This step includes verifying proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening comprises an initial rapid assessment for drugs of abuse. While it offers an economical method to exclude drug consumption for most samples, positive findings must be verified to be court-admissible. Presumptively positive samples from Screening require further confirmation.

Should a sample be presumptively positive in Screening, additional hair is extracted from the initial sample and readied for Extraction. Drugs are extracted from hair in this stage at lower concentrations than in other techniques (e.g., urine or oral fluid), making hair drug testing the most complex methodology.

Confirmation of any positive results from Screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation, as needed. The whole lab process, from Accessioning to Confirmation, complies with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
