For EtG, additional tests, or those beyond 10 panels, 150mg of the hair sample is advisable. We suggest using a jeweler's scale to weigh the specimen. In cases where scalp hair is not available, an equivalent amount of body hair may be obtained. When discussing head hair, we specifically refer to scalp hair. Body hair denotes other types (such as facial, axillary, etc.).

Process Overview

The drug testing lab processes involve four primary steps: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample undergoes initial processing to integrate into a lab's system. This step confirms the sample's seal and proper shipment, assigns it a random LAN (Laboratory Accessioning Number), and completes any needed data entry not covered by an electronic chain of custody.

Screening provides a preliminary assessment for potential drug abuse. While Screening is cost-efficient for discounting drug use in most samples, a positive result demands confirmation for legal substantiation. Samples with presumed positive results during Screening require further confirmation.

For any presumptive positive sample in Screening, additional hair from the initial specimen is prepared for Extraction. In this phase, drugs are removed from hair at a lower concentration than other methods (e.g., urine or oral fluid), thereby increasing the complexity of hair drug testing.

Positive results in Screening undergo Confirmation through techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. Samples presumed positive are washed before confirmation when necessary. The comprehensive laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
