What it is, How it works

Hair analysis is increasingly recognized as an effective means for detecting the use of substances like drugs and alcohol. Hair captures a long-term record of these substances by incorporating biomarkers within the strands of growing hair. When collected near the scalp, hair offers a detection period extending up to about three months for both alcohol and drugs. It is easy to collect hair samples, challenging to tamper with, and straightforward to send.

A sample measuring 1.5 inches, consisting of around 200 hair strands (comparable to a #2 pencil's thickness) closest to the scalp will provide 100mg of hair, the preferable amount for both screening and confirmation purposes. For EtG, additional tests, and/or panels exceeding 10, it is suggested to collect 150mg of the sample. It is advised to weigh the specimen using a jeweler's scale. If scalp hair is unavailable, gathering an equivalent quantity of body hair is permissible. The term head hair refers solely to scalp hair. Body hair includes all other hair varieties (facial, axillary, etc.).

Overview of the Process

Four fundamental steps are involved in laboratory analysis of drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is entered into the laboratory's system. This involves ensuring the sample was properly sealed and transported, assigning a random Laboratory Accessioning Number (LAN), and completing any additional data entry missing from an electronic chain of custody system.

Screening offers a preliminary assessment for substances of abuse. While Screening efficiently rules out drug usage for most samples at low cost, a confirmed positive result is crucial for court validity. Samples deemed presumptively positive during Screening require a follow-up confirmation.

If identified as presumptively positive during Screening, additional hair is taken from the original sample and readied for Extraction. Here, drugs are isolated from hair at far lower concentrations than in other sampling methods (e.g., urine or oral fluid), making hair drug screening particularly complex.

Confirmation of any positive screen result is completed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation when necessary. The entire lab procedure, from Accessioning through Confirmation, is subjected to review under both the CAP (College of American Pathologists) Hair criteria and the accreditation aligning with ISO / IEC 17025 standards.
