A sample measuring 1.5 inches of roughly 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal amount for both screening and confirmation. For EtG, extra tests, and/or panels exceeding 10, a specimen size of 150mg is advisable. It's best to measure the specimen on a jeweler's scale. If scalp hair is not available, an equivalent amount of body hair can be gathered. When referencing head hair, we mean scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in processing a drug test in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a lab's system. This step includes ensuring proper sealing and shipping of the sample, assigning an arbitrary LAN (Laboratory Accessioning Number), and fulfilling any additional data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid assessment for substance abuse. While it's an economical approach to exclude drug usage for most samples, a positive screening result requires confirmation for legal acceptance. Any samples that are preliminarily positive in Screening necessitate a follow-up confirmation.

If the Screening phase results in a tentative positive, additional hair from the original specimen is prepared for Extraction. Here, drugs are extracted from hair at significantly lower concentrations in comparison to other methods (e.g., urine or oral fluid), highlighting why hair drug screening is among the hardest methodologies to execute.

Confirmation for any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity undergo washing before confirmation if required. The full laboratory workflow from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and with accreditation to ISO / IEC 17025 standards.
