Hair gathered near the scalp can reveal a detection period of up to three months for alcohol and drugs. It's easy to collect, hard to tamper with, and simple to transport.

A 1.5-inch hair segment consisting of roughly 200 strands (similar in size to a #2 pencil) closest to the scalp provides 100mg, which is the optimal amount for analysis and confirmation. For EtG, additional tests, and/or evaluations exceeding 10 panels, it's advised to gather 150mg of the sample. It's best to weigh the sample with a jeweler's scale. If scalp hair can't be sourced, an equivalent volume of body hair may be used. Head hair refers exclusively to scalp hair. Body hair encompasses other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into the lab’s system, ensuring the sample's seal and proper shipping, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent in an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drugs of abuse. Although Screening offers a budget-friendly method to rule out drug use in most samples, a positive Screening necessitates confirmation to hold up in legal proceedings. Samples presumed positive in Screening must undergo subsequent confirmation.

For samples deemed presumptively positive during Screening, additional hair is extracted from the primary specimen for the Extraction phase. During Extraction, drugs are isolated from hair at much lower concentrations than in some other methods (e.g., urine or oral fluids), making hair drug screening the most challenging test to conduct.

Confirmation of positive Screening outcomes is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample is cleansed before confirmation if needed. The full lab sequence from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair certification and the ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
