Its collection is straightforward, challenging to tamper with, and simple to ship.

A sample of 1.5 inches comprising roughly 200 hair strands (approximately the diameter of a #2 pencil) nearest to the scalp provides 100mg, which is the optimal amount for both screening and confirmation. For tests like EtG, add-ons, or those beyond 10 panels, 150mg is preferred. We advise using a jeweler’s scale for weighing the specimen. In the absence of scalp hair, a similar quantity of body hair is acceptable. By head hair, we mean only scalp hair, while body hair encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The four crucial phases in the lab processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial intake of a sample into a lab’s system, ensuring the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and entering any further data not covered by an electronic chain of custody.

Screening is a preliminary, quick check for drug substances. Though cost-efficient for ruling out drug consumption in most samples, any positive screen requires confirmation for court legitimacy. Presumptive positive findings in Screening demand additional confirmation.

Should a specimen test presumptively positive during Screening, more hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower levels than alternative methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of positive screening outcomes uses GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive specimens are cleansed before confirmation as necessary. The entire laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair classification and accreditation pursuant to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
